Pharmacological Actions of Principal Ingredients 1. 5a-reductase enzyme inhibitory and a-adrenoceptor antagonistic actions: 5a-reductase inhibition blocks the conversion of testosterone to dihydrotestosterone, the major sex hormone in the prostatic cells responsible for BPH. Inhibition of 5a-reductase enzyme results in increased levels of testosterone, and possibly, increased levels of estradiol. The 5a-reductase enzyme inhibitor drugs are used in BPH, prostate cancer, and baldness. Himplasia inhibits 5a-reductase activity and has a-adrenergic antagonistic activity. Himplasia significantly reduces prostatic weight, epithelial height, and stromal proliferation. Tribulus terrestris is useful in BPH, genitourinary infections, painful micturition, hematuria, and dysuria. Crataeva magna and Areca catechu possess 5a-reductase inhibitory and a-adrenergic blocking activities, and thus, are effective in prostatic hypertrophy, neurogenic bladder, and chronic urinary infections. Cystometric studies have shown that Crataeva magna improves bladder tone and relieves post-prostatectomy atony of the bladder. Asparagus racemosus increases levels of estradiol, one of the important functions of 5a-reductase inhibitory activity. 2. Antioxidant action: Recent evidence suggests a relationship between oxidative stress and BPH. Tribulus terrestris, Caesalpinia bonducella, Areca catechu, Asparagus racemosus, Crataeva magna, Acacia nilotica, Aloe vera, and Dolichos biflorus have potent antioxidant activity. 3. Anti-inflammatory action: Various inflammatory cascades play an important role in the etiology of BPH. Tribulus terrestris, Asparagus racemosus, Crataeva magna, Acacia nilotica, and Aloe vera exert strong anti-inflammatory activity and have a beneficial effect in BPH. 4. Antispasmodic action: Acacia nilotica relieves the associated symptomatic pain by virtue of its antispasmodic activity.
