T h e r a p e u t i c I n d e x 94 – Mucus colitis • Miscellaneous: – Acute pain associated with lower UTI – Ureteric spasms – Chronic pancreatitis Dosage Abdominal spasmodic pain: 2 soft gelatin capsules to be taken as soon as the acute spasmodic pain is felt, followed by 1 soft gelatin capsule every 8 hours for 3 to 5 days. Spasmodic dysmenorrhea and menstrual cramps: 2 soft gelatin capsules twice daily till the menstrual cycle completes. Irritable bowel syndrome (IBS): 2 soft gelatin capsules twice daily for 4 to 6 weeks. In other types of colics including biliary and renal colic: 2 soft gelatin capsules stat. Repeat if necessary. Himcospaz may be combined with other conventional antispasmodics. Adverse Effects No adverse effects have been reported. Contraindications Himcospaz is contraindicated in the last trimester of pregnancy, as it inhibits prostaglandin- and oxytocin-induced contractions (as observed in in vitro experiments), which may prolong gestation and labor. Presentation Box of 10 blister-pack strips of 10 soft capsules each. Pharmacological Actions of Principal Ingredients 1. Antispasmodic action: Himcospaz inhibits the contractions induced by several spasmogens (such as acetylcholine and histamine), and acts by inhibiting the calcium influx from sarcoplasmic reticulum. Zingiber officinale inhibits both acetylcholine-evoked and electrically-induced smooth muscle contractions. The spasmolytic action is attributed to gingerol, an active constituent of Zingiber officinale, which also inhibits prostaglandin biosynthesis and has serotonergic activity. Foeniculum vulgare acts synergistically in controlling colicky pain. Apium graveolens and Curcuma zedoaria have spasmolytic effect, especially in the gastrointestinal tract. 2. Pain-relieving, anti-inflammatory, and antioxidant actions: Prostaglandin inhibitors are effective in the treatment of dysmenorrhea and other smooth muscle colics. Apium graveolens, Curcuma zedoaria, and Zingiber officinale have pain-relieving, anti-inflammatory, and antioxidant actions. Gingerol from Zingiber officinale also acts by interfering with the intracellular signaling cascades (those involving NF-kappa B and mitogen-activated protein kinases). Himcospaz® (soft capsule)
